Notes: If you have indicated that you require program equipment for your event/activity, it is your responsibility to contact one of the following individuals at least two (2) weeks prior to your activity:

Failure to notify one of the above of a cancellation within 48 hours of your requirement will result in a mileage fee being charged to your Group/Section.
